HPI: SP is a 21-year-old woman who presents with a 3-day history of a painful sore throat. She has pain with swallowing and even with talking. She also has felt feverish for the past 2 days and somewhat listless. She does not have a cough, runny nose, or shortness of breath. She just started a new job as a nanny and notes that the 18-month-old baby has been fussy, feverish, and not eating well.
PMH: Had eczema as a child. Takes no medications and has no drug allergies. Not sexually active.
PE: T 38.4°C; HR 88; BP 100/70; RR 12
On oropharyngeal exam, her posterior pharynx is erythematous, swollen, and coated with a whitish exudate. Bilateral tonsils are also edematous, red, and covered with a similar exudate. The lymph nodes in her neck are swollen and tender just beneath the angles of the mandibles. Lung exam is clear.
Labs: WBC 10.0 with 80% neutrophils. Throat culture pending.
